Patient Instructions for Medical Record Requests

South Shore Orthopedics has partnered with HealthMark Group to ensure the accurate and timely completion of medical record requests.

How?

Requests may be submitted electronically to HealthMark’s Request Manager at https://requestmanager.healthmark-group.com. Once logged in, select “Submit Request” from the menu options and enter all required fields to provide an authorization directly to HealthMark. Your medical record request will be processed, and a notification will be sent via mail or email once complete and available for download.

X-Ray Requests

South Shore Orthopedics is happy to offer patients the ability to view your X-rays online in the comfort of your home.  No more burning cd’s or waiting for the mail. Welcome to Exa PACS, our portal so you and your doctors can view your images. If you have an email address and want to be able to look at your X-ray images, this was made for you.

How to Register for Your Exa PACS X-Ray Portal:

Alternatively, please email us at [email protected] to request Exa access and we will send you a registration link. If you have not received yours within 48 hours, please email us back or call 781-337-5555.

FAQ

What if I don’t have an email address?
If you do not have an email address, you will not be able to use the portal at this time.

What if I forgot my password?
If you forget your password, you can reset it from the sign-in page by selecting FORGOT PASSWORD

Can I have an authorized representative view my X-rays for me?
Yes, you may appoint an authorized representative in your profile. Authorized reps need to have an account as well.

Does this only include new images?
No, prior images taken at our offices from roughly 2012 onward are accessible, as well as images taken at South Shore Health from 2022 onward.
